Our Behavioral Health Integration (BHI) program is a monthly, telephonic care coordination service. Download our referral form on this page by clicking on it, take it to one of your doctors and fax it back to us.
A member of our staff will verify your insurance, your diagnosis and obtain your verbal consent.
With our BHI program, you have access to social workers, nurses and psychiatrists who are all on our clinical team.
Your designated clinical care team member will work with you to create a care plan and outline goals to work towards. This care plan gets reviewed by a psychiatrist on our staff.
Your care team member will:
Link you to resources or other behavioral health professionals
Provide you with additional support
Coordinate your physical and mental health care needs
Help you learn and implement new coping strategies
Identify ways for you to successfully accomplish your goals
Our program is entirely telephonic, allowing patients to schedule phone calls with our care team when it is most convenient for them.
We can work with people of all ages. With children and adolescents, we work alongside the patient as well as their parent and/or guardian. With patients over the age of 18 who are not able to make their own medical decisions, we can work directly with a caregiver.
